What To Expect At Your First Appointment
Please arrive 10-15 minutes before your scheduled new patient appointment so that you can fill out your new patient paperwork. When you enter the office, let the receptionist know you are here for a new patient appointment. The receptionist will then have you sign in for your appointment and will need to make a copy of your insurance card and driver's licence (and your prescription insurance card, if separate from your health insurance card.) The receptionist will give you your new patient paperwork (unless you have already downloaded it below and filled it out) then collect your co-payment. After you have all of your paper work filled out and you are checked in, the nurse will take your vitals and place you in an examination room. Then the doctor will be with you shortly! New Patient Information
If you are a new patient, we have some forms for you to fill out! If you'd like to download and fill them out ahead of time, you're welcome to do so. If not, we will be happy to give them to you when you arrive. Remember, make sure to bring the forms with you and give them to the receptionist, along with your driver's license and insurance card (and prescription insurance card, if it is separate).
In order to monitor your health as efficiently as possible, it is important for the Doctor to know what medication you are currently taking, as well as medications you have taken in the past. This form will allow you to write down your medications at home, where you have your prescriptions readily available!
